The NCHC works with community coalitions, agencies, and collaborative groups to improve the health of our communities. Using the latest prevention research, data, curricula, and tools, the NCHC can work with your group to: 

  • Develop effective coalitions.

  • Identify community assets and needs.

  • Collect and analyze community health data.

  • Lead strategic planning processes.

  • Develop and implement health promoting programs and activities.

  • Plan and conduct program assessments.

  • Evaluate the planning process.

The NCHC promotes the Healthy Communities Approach to prevention, which takes a broad view of health and focuses on the total community as the ideal context for health promotion including the social, economic, political and geographic environments. Focusing on systems change, the Healthy Communities Approach seeks to build broad participation, encourages new players, honors diversity, creates a shared vision and values, and builds true collaborations that utilize the assets and resources of the community. Integral to this approach is the measurement of results and the establishment of benchmarks for progress.
