Community Needs and Resource Assessment

A formal needs and resource assessment identifies the nature and extent of health problems in your community as well as determines what resources are available and/or needed to address the problem. It is a roadmap for change and serves as the basis for the strategic plan that will address specific problems in your community. The NCHC can assist your group or coalition to determine the appropriate indicators to collect, locate proper data sources, compile the information into a report, develop a dissemination plan as well as identify priority issues and formulate goals and strategies.

Health Surveys

The NCHC can assist your group or coalition with all aspects of conducting local health surveys.  The NCHC staff can work with you to:

  • Locate a survey tool that will fit your community.

  • Locate, administer and analyze a youth survey for your community or school district including the Youth Risk Behavior Survey.

  • Design appropriate survey tools and pre- and post tests.

  • Analyze and interpret data.

  • Develop recommendations.
